Camp Waiver Information
Athletes are required to complete the Physician’s Clearance Form (link immediately below) and provide proof of insurance with front and back copies or pictures of their insurance card. Close, clear pictures of these documents can be uploaded during the online registration process or by returning to the Upload Required Forms tab on the camps website after the completion of online registration.   Both proof of health insurance and the completed physician’s clearance form are REQUIRED for ATHLETE participation. If you have attended one of our camps in the previous 6 months, you can use those documents in lieu of resending them again. Just let us know via email. Coaches do not need to complete this form or provide insurance information.
